making an application for an authorised supervisor
All children's services staff, in every type of children's service, must be aware of their obligations as mandatory reporters and must have completed the required training. For detailed information on these requirements and the relevant legislation, see DoCS Mandatory reporters page.
The authorised supervisor is responsible for the day-to-day running of the service and has specific regulatory responsibilities. These are summarised in what is the role of authorised supervisors?
With your licence application you (the prospective licensee) must include completed supervisor nomination by licensee and screening consent forms, and:
- provide documents that demonstrate that the nominated person is eligible to hold this position:
- certified copies of the person’s qualifications (witnessed by a Justice of the Peace or legal practitioner)
- work references from 2 independent referees
- character references from 2 independent referees each dated no more than 3 months before the reference is provided to the Director-General (ie. the date you make your licence application)
If a licensee intends to change the authorised supervisor, then completed supervisor nomination by licensee and screening consent forms must be submitted.

When processing your application DoCS will:
- check that all the required documents have been supplied with the application, where applicable. You cannot appoint anyone as an authorised supervisor until DoCS has determined that person’s suitability for the authorised supervisor’s role. If the authorised supervisor is also a member of the primary contact staff (refer to the staffing section of this website), they must also meet the requirements of primary contact staff.
- check whether the proposed authorised supervisor’s qualifications are approved or not approved.
If you have a qualification that has not yet been approved it will need to be formally assessed by DoCS.
