licensing forms and guides
Children's services has a new IT system that will provide a faster, more efficient system for licensing of children’s services.
The process for applying for a licence remains the same, but there are new forms that will simplify applying for a licence or a licence variation.
Note: You do not need to resubmit your application on a new form if you have already submitted it.
applying for a licence
Choose the forms for your situation.
- a new children's service
- further licence for an existing service
- change of provider for an existing service
- variations to a licence
- notices of changes
- out of school hours care
- child-minding in a retail shopping centre
- early childhood teacher
- qualifications assessment
tips to help you with the new forms
- Be sure that you are using the current forms. Do not use forms which you have printed off earlier
- Make sure to use the correct forms for your situation:
- I want to apply for a licence for a children’s service which is not currently licensed. See new licences.
- I am going to be the new operator of an existing licensed children’s service. See change of provider licences.
- I am the operator of an existing licensed service and want a further licence for the service. See further licences.
- If the service is provided by a school make sure that you always use the forms listed under the heading ‘children's service provided by a school'
- If the licensee is to be one individual (a ‘flesh and blood’ person such as Mary Smith or Bill Black) use the forms for ‘if service provider is an individual’
- If the licensee is to be a partnership or an organisation (a company or incorporated association) use the forms for ‘if service provider is an organisation or partnership'
- Be sure to lodge each form with all the documents which the form requires. If for some reason you cannot lodge one of these documents with the form attach a note explaining why
